伤心 (shāng xīn) is deep sadness, the kind that comes from loss, betrayal or a hard goodbye. It is stronger than 难过 (nán guò), which covers ordinary being-upset, so save 伤心 for real grief.
Grammatically it behaves as an adjective and takes 很: 我很伤心. It also works in a causative frame with 让 or 使: 让他伤心 (to make him sad), which is how you say "you hurt his feelings" without any word for hurt.
The trap is the literal reading. 伤 does mean wound, but 伤心 is never physical. For a physical injury to the heart or anything else you want 受伤 (shòu shāng, to be injured), a completely separate word.
