c'est or il est?
Both are he is or it is. What follows decides which.
The rule
c'est is followed by a NOUN or a noun phrase: c'est un médecin. il est is followed by an ADJECTIVE or a bare profession with no article: il est médecin, il est grand. The article is the tell, and English gives you no help at all here.
