Extra takes the accusative and nothing else. As a bare adverb it means simply "outside", with no object to govern, and that is the use in extra stare.

Its exact opposite is intra, "within", and the two map a boundary between them. Ultra is a different idea: it measures distance past a line rather than being on the far side of an enclosure. Set phrases carry a lot of the word weight - extra ordinem, "irregularly, out of turn"; extra iocum, "joking aside"; and the canon-law tag extra ecclesiam nulla salus. From the adjective extraneus, "outside, foreign", English took "extraneous" and, through Old French estrange, the word "strange".
