Requiro, requirere, requisivi (or requisii), requisitum, third conjugation. It is re plus quaero, with the a of the simple verb weakening to i in the compound, the same shift that turns facio into conficio and capio into accipio.
Three senses, and the third catches people. It searches for a thing, it enquires after information, and it feels the absence of something that ought to be there: amicum requiro is "I miss my friend", not "I am hunting for him behind the sofa". Medieval Latin adds a fourth, the bureaucratic "require, demand", and that is the one English inherited, along with request, requisite and requisition. The participle requisitus is by far the commonest form in this corpus.
