One root covers the ache in your knee and the grief in your chest, so context does the sorting, exactly as it does for English "pain". The verb behaves the way a Romance speaker expects and an English speaker does not: it is the body part that hurts, doloras , rather than you doing the hurting. Dolora is painful, i is to cause pain deliberately, and sendolora is painless.
