is a twin, and it works for either sex; o exists for a female twin when the distinction matters, and covers a pair of any composition. The plural does most of the work in practice: is how you say they are twins, and is the fuller phrase when you want the relationship spelled out.
The adjective has a life beyond biology. is a twin town - the civic-partnership sense, which Esperanto arrived at by the same metaphor English did. are word pairs, and in mechanics are twin wheels.
Watch the root boundary. Ĝemi is to groan or to sigh, and - looks like ĝem- with something added. It is not: there is no suffix -- in Esperanto, and the two roots are unrelated - one Latin, one imitative. A twin who groans is a , which is a sentence worth saying slowly.
