出生日期 (chūshēng rìqī) is the formal term for "date of birth", the phrase you meet on forms, passports and official documents rather than in daily chat. 出生 (chūshēng) means "to be born" and 日期 (rìqī) means "date", so the compound is literally a "birth date".

Do not confuse it with 生日 (shēngrì), "birthday", which is what people actually say when wishing someone well each year; 出生日期 (chūshēng rìqī) is bureaucratic rather than celebratory, so you would never hear it in a birthday greeting.

Because it is a fixed four-syllable phrase, it always appears as a whole unit; do not try to split it or insert other words into the middle of it.
