Nobody knew about it except her.
is genuinely two-faced: it means both "except" and "in addition to", and only the sentence around it decides which. In practice a negative or an exhaustive word nearby pulls it towards exception, as in or , while an in the main clause pulls it towards addition. That is why X, Y is the safe pattern when you mean "as well as". It takes the nominative, and introduces a whole clause.
