a deux sens principaux selon le contexte : always (habitude constante) et still (une situation qui continue). Ainsi il est toujours là peut signifier "he's always there" ou "he's still there" - l'ambiguïté se lève par le contexte ou par l'ajout de encore pour still.
La locution toujours est-il que (the fact remains that) introduit une conclusion malgré les réserves précédentes. Pour toujours (forever) exprime la permanence absolue : partis pour toujours. L'antonyme est jamais, qui exige ne dans une phrase négative complète.
